The process of care in managing people with type 2 diabetes: What is the impact of peer support on uptake and effective implementation of care plans in the self management of diabetes?

This research aims to determine whether and how a peer support program impacts on initiation and use of care plans by Australian GP's.  It will examine whether the use of formal care plans is related to health outcomes and identify ways in which management plans and team care arrangements can be used more effectively in managing the care of people with type 2 diabetes (T2DM) in the Australian community.
