Chronic Disease Management of Co-morbid Depression, Heart Disease and Diabetes - Stage 1
This project is examining the clinical pathways for patients with co-morbid depression and diabetes using six general practices in the National Primary Care Collaboratives, which are required to set up chronic disease management systems with databases for recall. The project will also seek to determine where best to identify and intervene for patients with diabetes and depression.
This project is funded by the beyondblue Victorian Centre of Excellence in Depression Related Disorders and is a collaboration between the GGT UDRH and the University of Melbourne.
