Evidence-based Best Practice Model Clinical Pathways for People With Diabetes

The main objectives of this project are:

  1. To quantify hospital and other medical service utilisation for diabetes and diabetes complications
  2. To audit diabetes management against the guideline: Diabetes Management in General Practice, Diabetes Australia and RACGP 2005/2006
  3. To identify the psychosocial risk factors of depression and social isolation
  4. To audit diabetes management systems against recommended models
  5. To identify systems barriers and enablers to optimum diabetes management
  6. To identify causes of preventable hospital admission for complications of diabetes
  7. To identify key stakeholders and participants able to consolidate, expand and strengthen partnerships
  8. To identify and test solutions, including clinical pathways, to reduce the high admission rates for complications of diabetes
